The Blazer Foundation was established by Cedric W. Blazer to improve the lives of individuals who have mental or physical disabilities.

How to Apply

If you are an organization in Winnebago County or adjoining counties in northern Illinois helping individuals who have mental or physical disabilities, you may qualify for The Blazer Foundation funding.
